Co-pays, deductibles, co-insurances and fees for non-covered services are expected to be paid at the time of your service.

For your convenience we accept cash, personal checks, Visa, MasterCard, American Express and debit cards.

How to Qualify for Sliding Fee Scale

Our services are not free, however they are comprehensive, high quality medical services that are provided regardless of a person’s ability to pay for them at the time of service.

Once you arrive, you will be directed to our Patient Account Counselor who will walk you through our Sliding fee program. We use a scale based on family size and income to determine patient’s share of the cost of care.

To be placed on the sliding fee program, please bring proof of your income (a pay stub, W2 or letter from your employer). Your fee level is assessed and based on your family size and yearly income.

The total cost of your service may consist of the following fees: Office Visit, Procedure(s), if necessary, Prescriptions, if necessary, Labs, if necessary and Injections for Podiatry, if necessary. Please download a schedule of estimated fees for 2019.
