FAIRMOUNT PRIMARY CARE CENTER AT HORIZON HOUSE

DVCH and Horizon House, Inc. partnered in 2006 to create the Fairmount Primary Care Center at Horizon House (FPCC-HH), a unique collaboration to address the medical, psychiatric, behavioral and dental health needs of individuals with disabilities as well as those who are or have been homeless. FPCC-HH is open exclusively to Horizon House Program participants and staff members. FPCC-HH patients also have full access to the wide range of care services such as Dental, Pediatrics, Health Education, and Social Services available at Fairmount Primary Care Center, 1412 Fairmount Avenue. Patients can walk-in or call 215-235-9600 to schedule an appointment.
